B5004 was constructed by Sharp Stewart (2936/1880). It was originally Staats Spoorwegen 39 and then 221. This 2-4-0 had two outside cylinders 381 x 457 mm with driving wheel diameter of 1413 mm. Boiler pressure was 10 kg/cm2, grate area 1.1 m2 with a heating surface of 50.0 m2. Overall weight was 20.3 tons with 15.3 tons available for adhesion. B5004 is preserved at Taman Mini near Jakarta. In the 1970s this class was almost entirely based at Madiun for the branch line to Slahung although a couple of examples were kept at Jatibarang where they had worked the Indramayu branch but were then only used for pumping.
